Please read these studio policies and eligibility requirements before scheduling your appointment.

Booking policies

Your time matters. So does the time set aside for your appointment.

Deposits and rescheduling

Booking deposits are non-refundable. You may reschedule once with at least 48 hours’ notice without forfeiting your deposit. Additional reschedules require a new deposit. Cancellations with less than 48 hours’ notice do not receive a refund.

Arriving late

Please email as soon as possible if you expect to be more than ten minutes late. Arriving over ten minutes late may result in cancellation and loss of your deposit. Whether the appointment can proceed is decided case by case according to the schedule. If it cannot proceed, a new deposit is required to reschedule.

Previous permanent makeup

Before booking, email a clear photo without makeup, looking directly into the camera, to sam@forever-flawless.ca. Samantha must approve existing work for a cover-up. If you arrive with previous PMU that has not been approved, your service cannot proceed and your booking deposit is forfeited.

Initial and perfection sessions

Touch-ups are not included in individual initial session prices and must be booked separately. The $1,250 + HST brows, lip blush and lash enhancement package is the confirmed exception: it includes perfection sessions for all three areas.

Preparation and aftercare

Read your service’s preparation, aftercare and eligibility information before booking. Failure to follow preparation instructions may mean your appointment is cancelled and the booking deposit is not refunded or transferred.

Free consultations

A consultation is for discussion and professional guidance only. Neither you nor Samantha is obliged to proceed with a treatment booking. A no-show without notice, whether virtual or in person, results in being blocked from future consultation or service bookings. Please give adequate notice if you cannot attend.

Before you book

Studio eligibility requirements

Please review these requirements before scheduling a treatment.

Do not book while pregnant or breastfeeding.
If you take blood thinners, speak with your prescribing clinician and Samantha before booking. Any medication changes, including stopping or restarting, must be directed by your prescribing clinician.
Do not book if you have had Accutane treatment within the past six months.
Allow at least two weeks between Botox or filler injections and a permanent makeup procedure.
Glaucoma excludes permanent eyeliner at Forever Flawless. Other services remain subject to the remaining eligibility requirements and individual assessment.
Previous permanent makeup requires photo assessment and Samantha’s cover-up approval before booking.

If you have a cold-sore history, discuss an antiviral plan with your prescribing clinician before lip blush and let Samantha know.

Booking forms on this website are provided by Acuity Scheduling. Information you enter in those forms is submitted to Acuity.
